- Photo ID (driving licence or passport)
- Glasses/contact lenses and your most recent prescription (if applicable)
- Details of any medical conditions and regular medication (if applicable)
- Urine analysis (if required by employer)
Motorsport Medicals
- Photo ID (driving licence or passport)
- Glasses/contact lenses and your most recent prescription (if applicable)
- Details of any medical conditions and regular medication (if applicable)
- You will need to provide a urine sample at the clinic
- Download a Motor Sport Association Medical form
- Please note that this is for a standard Motorsport UK medical only and does not include an ECG test

- Blank Taxi Medical report form(of Council Authority you are applying for)
